Treatment of cases where the symptom picture is unclear
In the organon Hahnemann gives us lots of teaching on how to go about such cases.
In Stanza 149 it says that diseases of long standing will also take a long time to cure. But then we need to know our remedies well to make the best choice so cure can happen quickly instead of chasing remedies for a great length of time, thereby extending the time of cure. It is also possible that the case has not been properly taken and without the best symptoms we cannot get the best rubrics or the simillimum.
In stanza 174 we note that some diseases can be physical symptoms only - called local maladies and then there are those that have only mental symptoms (often one sided diseases and rare).
Often a chronic case is complicated by an acute phases - always treat the uppermost sx and separate out the acute from the chronic - do not mix the two together.
An allopathically spoiled case as we can read in stanza 91 has to be brought back to its beginning so as the see the real and true picture.
Some cases can have altering states which seem complicated and difficult to see through and understand fully but there are many remedies that cover these types of cases.
And where disease states have succumbed to deep mental pathology the case has to be looked at from the point at where one can see what was going on before this - stanzas 216-218, that is, to be able to see the original case that had a host of physical symptoms as well as all the others.
One sided case are rare but if you do have to treat one then there will be keynotes to use especially if they are strong and characteristic then a cure is likely.
The controversial ‘layer’ approach is hinted at in stanzas 165 +. Today this has led to considerable lazy homeopathy in my opinion because it means that practitioners have misunderstood the points he makes - just because a case is unclear (for many reasons) it doesn’t mean to say we have to prescribe on bits and pieces, which will only complicate the case more.There were many fewer remedies then as well but we have plenty of remedies now to cover almost all aspects of any disease and it is up to us to persist with a better way of taking the case. However if, after a thorough case taking there is still a weak picture but one that can be prescribed on then you have to go with what you’ve got but this again is rare and shouldn’t be relied on so as not to try harder. If taking this approach and the symptoms change and they do not belong to the remedy then the fuller case can now be analysed better with more symptoms to work with.
If you do have an unclear case with not many symptoms then it is imperative that the case be graded as to the importance of the symptoms - stanza 91 - of course you have to get this correct otherwise you will give weight to a common symptom and maybe lose the really important one or two.
Topical employment of medicines is inadmissible - stanza 198 - it might remove the chief symptoms but will undermine the others and cure will never be found with such an unclear picture. This can be illustrated by the use of poor choice intercurrent such as so called organ support remedies or so called tissue salts.
Stanza 94 tells us about maintaining causes which can also make a case unclear - they should be removed before a true picture can be relied upon or if confident of the chosen remedy it might need a more frequent dosing to deal with the ongoing maintain causes.
