Preamble and first observation
In May 2012, the parents brought a 5.5-year-old girl who has had constant problems with eczema for two years, and since two months ago eczema behind the right ear. I looked, removed the hair that obscured the eczema, and saw an oval change below the very margin of the hair, irregularly shaped, about 2 cm in diameter; the entire surface of the skin was peeled, with one part (approximately a quarter of the surface) where there was a shallow, open wound. All around and spreading more towards the head and going under the hair, the skin was covered with spotted white changes that were flaking. The girl complained of severe itching, she scratched intensively, and scabs would appear in those places. I also noticed that the surface was greasy and asked what it was. The father replied that since the appearance of eczema, they have been using various ointments based on corticosteroids prescribed by a dermatologist, but none of them helped. On the contrary, eczema is persistently spreading. I gave the suggestion that as soon as come home, they wipe off the ointment, not to apply it any more, and I scheduled a consultation.
Taking the case
The girl was born from a normal pregnancy and delivery. At the age of 28 days, she was hospitalized for pemphigus. The mother described the changes: "The whole buttocks were covered with small inflamed blisters and the infection spread towards the navel." She cried a lot. She was treated with antibiotics and corticosteroids. A year and a half before they visited me, a red seal appeared on her buttocks and the skin was rough at that point. She was treated with corticosteroid ointments. In general, she is a healthy child with an occasional cough accompanied by a temperature that usually rises to 39 degrees C.
The father said that the girl is very naughty. She likes to eat a lot, sometimes behaves churlish and rudely; everything has to be her way and if it isn't, she becomes hysterical. Sometimes, she can pleasantly surprise them, wants to help with household chores and to be obedient. When asked what he means with 'hysterical', he said that she is arguing with his mother and family members, shouting and screaming. She used prefer to cuddle, not anymore.
When asked how much it itches, the girl answered that it itches occasionally, it makes her a little better from scratching, but later it itches again. (She allowed me to take photos and was willing to cooperate. In the meantime, she drew a house, flowers and wrote her name on the drawing.) She has one best friend, they play with dolls together, but 'she is rude and sometimes tries to tear my doll.' "Then I hit her, and sometimes she hits me too." She said her brother was boring and wanted her to take her toys. When asked what then she's doing, she said: "I hit him!" She said that, like for a friend, childishly honestly, with a smile of unpleasantness, but also with self-confidence. When asked when else is she angry, she said: "At grandma, because when I go to her house, she says: Go home!"

- Then I start crying and shouting "I don't care!" And then I go home.
- Besides, when else are you crying?
- Only when I'm angry.

- I'm never sad, I'm just angry.
- What are you afraid of?
- Of everything! Dogs on the street, cats, pigs. I'm afraid of beetles: ants, spiders, ladybugs. Darkness.
- Why?
- Dogs and pigs because they want to bite me. When I go to the chicken coop, the cat comes after me. That scares me. When dad and mom turn off the light while I'm alone in the room. When they are in the room, then I am not afraid.
- What are you most afraid of about cats?
- When a cat walks past me and touches me with its tail – it tickles me and it’s so soft. (On her face could bee seen disgust and fear.) The tail is soft, and the hair. I get scared and scream.
- And what about ladybugs?
- I'm scared when a beetle creeps on my arm, as if it wants to bite me. Ants are the scariest!
In the continuation of the conversation, the mother told me that she had never seen her daughter sad. She is always angry and furious, even when she is sick and has a high fever. She also said: "She always gets what she asks for because she is so persistent that in the end we have to give it to her." When asked how she manages that, she said: "She uses all means, just to get what she wants. She is sitting next to me and doesn't stop asking until I give it to her."
Analysis

- extremely intolerant to touch: has a pronounced fear and aversion to touch (a cat with soft hair) and fear of insects;
- when she is very scared, screams;
- irritable and angry even when she is sick.

Already during the analysis and repertorization, I had a dilemma between Cham and Lyc. Irascible, angry, irritable, stubborn, hypersensitive to touch, aggressive, screams when angry, rashes, eczema – all these are covered by both remedies.
On the other hand, in the rubric: Mind; irritability; children, in; sick, when – there are only two remedies: Lyc and Puls. But, in the rubric: Skin; eruptions; pemphigus; newborns, in – Cham is one of 14 remedies, and Lyc is not one of them.
Due to the characteristics of the physical symptom and the strong tendency of Lycopodium for eczema in the area of the ears, especially behind, as well as on the margins of the hair, mostly on the right side, I opted for Lyc.
First prescription:
May 23, 2012: Lycopodium 30C, water solution, 10 drops
Course of treatment
May 30, 2012 – The area affected by the change has expanded slightly but became thinner and is no longer convex as at the beginning; almost completely leveled with healthy skin. It is much less itchy and the mother says that the first few days after taking the remedy, the child was much calmer and more obedient. One red dot appeared in the right earlobe. – without remedy
June 10, 2012 – At the root of the hair, on 4-5 times smaller area than before the treatment, there are spotted white scurfy changes. The skin affected by eczema is completely healthy, without any pathological changes. There is still a red dot in the right ear, unchanged in appearance and size. Occasionally itches a bit. She is often furious and angry again. – Lyc 30C, 1 dose
July 14, 2012 – The mother reports that the girl is coughing, immediately after returning from the sea. At the place where there was eczema, changes of elliptical shape, size approximately 0.8 x 0.3 cm, yellowish in color, slightly convex, looks like lichen. The child denies itching. – 3 doses of Lyc 30C were given, but the cough did not stopped.
Cham 30C, split dose – A day later: cough is less frequent and easier, occasionally spits mucus. The cough completely stopped within a few days.
August 15, 2012 – The mother calls after another stay at sea. A few days earlier, she noticed spot changes in both ears. During the consultation, I noticed the following: in both ears, white, scurfy, separated changes, the size of larger dots, without signs of inflammation and redness. The child complains of itching. – Lyc 200C
August 22, 2012 – The child does not scratch or complain of itching, there are 2-3 very small, red spots left in both ears. – without therapy, wait and observe
October 1, 2012 – Tiny red dots reappeared in both ears. – Lyc 200C
March 5, 2013 – Several white scurfy dots in earlobes, and behind the ears there are whitish changes, rough to the touch. – Lyc 200C
March 21, 2013 – The ears are completely clean, the skin is healthy, with no visible pathological changes. Parents say with a smile that she sometimes gets angry, but generally is much calmer and less defiant than before. She attends a preschool institution, where, as the girl says, she plays and hangs out nicely, and she's looking forward to start attending school in September. – no remedy
October 31, 2013 – White scurfy changes in the right ear, itch a bit. – Lyc 50M, half a teaspoon of water solution
Prognosis: In the next period, in this girl it might be needed to repeat the remedy in the spring and autumn, when the body significantly changes its work regime. This was also shown by the experience with her so far, considering that after the first complete relief from the symptom which lasted for several years, the changes reappeared at the beginning of October and the beginning of March. She is only less than 7 years old and her organism will develop for years to come.
If she had remained on corticosteroid ointment therapy, the body would have laid down its arms at one point and the eczema would have disappeared. Her dermatologist would say she was cured. It is very likely that at the first strong stress she would experience (most likely hormonal, when getting menstruation) she would start getting the first asthma attacks, because the suppression of many skin issues, especially in children:
1) forces the organism to appear with new symptoms, from deeper levels that then already are affected by the disease;
2) there is a direct link between suppression of eczema in children with the appearance of asthma in later periods of development.
Until August 2016th, sometimes I saw the mother at her workplace. Occasionally there were dots in the ears, but after a while they disappeared. She has not received any remedy since October 2013th.
They never called again because of any problems, so the logical conclusion is that the child is completely cured.
